Síndrome posagudo de COVID-19: una perspectiva vanguardista ante el efecto rebote de la COVID-19 en un caso señero en Puerto Rico

Resumen

ANTECEDENTES: Las guías médicas internacionales actuales no establecen una metodología adecuada para hacer frente al efecto multiorgánico negativo de la COVID-19 en los seres humanos. Mas allá de los tratamientos farmacológicos existentes en el mercado, es evidente que la atención médica de estos pacientes debe individualizarse conforme al contexto y la situación macroeconómica-social del lugar donde ocurra la infección. De igual manera, el seguimiento de estos pacientes es escaso y el fenómeno del síndrome posagudo de COVID-19 es mal diagnosticado.

CASO CLÍNICO: Paciente femenina de 66 años con diagnóstico de síndrome posagudo de COVID-19. A pesar de haber muerto debido a complicaciones, fue tratada de modo adecuado.

CONCLUSIONES: Este paradigma permitió reconocer cuáles son los elementos clave para llegar al diagnóstico certero e impulsar un tratamiento que permita ayudar a los pacientes que tuvieron COVID-19 y que recurren con afecciones multisistémicas, ante la escasez de información que permita a los médicos tratar adecuadamente a los pacientes. Creemos firmemente que este caso nos ha permitido identificar el procedimiento adecuado para tratar este síndrome en un hospital estadounidense con los recursos adecuados, pero también en otras instituciones con recursos limitados, como los de América Latina, debido a la simplicidad del método utilizado y la eficacia del tratamiento.

Abstract

BACKGROUND: Current international medical guidelines do not establish an adequate way to deal with the negative long-term impact of COVID-19 on multiple organ systems. Beyond the existing pharmacological therapies on the market, it is evident that the management of these patients is currently individualized due to the context and the macroeconomic/social situation of the place where the infection occurs. Similarly, the follow-up of these patients is poor and post-acute COVID-19 syndrome is often undiagnosed.

CLINICAL CASE: A 66-year-old female patient who was diagnosed with post-acute COVID-19 syndrome. Despite having died from complications, she was medically and pharmacologically adequately treated.

CONCLUSIONS: This paradigm made it possible to recognize the key elements to carry out an accurate diagnosis and a treatment that will allow proper patient management in individuals who had COVID-19 and recur with multisystem conditions. We firmly believe that this case has allowed us to identify the appropriate procedure to treat this syndrome in an adequately resourced US hospital, but also in any other institutions with limited resources, such as those in Latin America due to the simplicity of the method used and the effectiveness of the treatment.
